Leaving hospital is an important step in a person’s recovery, but returning home can sometimes feel overwhelming. Patients and their families may be concerned about personal care, medication routines, mobility, daily activities, community access, or managing ongoing support needs.
ABE Nursing & Support Services helps make this transition safer and more comfortable. We work closely with eligible individuals, families, hospital discharge teams, social workers, allied health professionals, and support coordinators across Adelaide to plan a smooth move from hospital to home or community living.
A safe discharge is about more than a ride home. It’s about making sure the person has the right support to follow their care plan, manage daily activities, attend follow-up appointments, and rebuild their confidence and independence.
When support is arranged early, patients and families feel more prepared for what comes next. Clear, ongoing communication between the patient, their family, the hospital team, and the service provider keeps care consistent and reduces stress during the transition.

Start dates depend on your assessed needs, funding approval, and our availability. We recommend getting in touch as early as possible during the discharge planning process so support can be arranged in time.
Our hospital-to-home nursing support may be funded through the NDIS, depending on your plan and eligibility. We do not provide My Aged Care services. Private and self-funded arrangements are also available.
